I know how much it sucks.
I found planning and time blocking helpful. even if you know what you have to do if it's hard you'll procrastinate. only if you pick a time and force yourself to do it then you'll do it.
sometimes it's hard to force yourself to sit, I use the Pomodoro technique for that. You work for a period of time after that you can do something you enjoy :)
