I need a tool to Transcribe Meeting Discussions in real time (for face to face meetings)

I have a full time job in an enterprise company, and we regularly have face to face meetings. I have googled for a solution but found none that meet my requirements. Requirements: - Identifies the voice of each individual. - Realtime Transcripts to go back through when misunderstandings arise during and after the meeting. - Writing up some notes on something (you can confirm) that was said in the meeting. - Separate and Summarise the specific discussions.
