Ideas on making interviews effective

I'm looking for ideas on standardising the interview process and ways to alleviate personal biases and opinions while talking to a candidate. I feel that as long as few people are doing the interview, these problems don't arise but as soon as more people get involved, the interviews and the results both become unpredictable. Your thoughts ?
