Looking for a no-code tool to do…

I’m looking to create simple web tool(s) for the autistic and adhd community. They are self assessments of sort that instantly gauge certain behaviours, emotional states and roadblocks to achieve daily living goals. I have seen grid.is and spreadsheetweb but it’s gets pretty expensive pretty fast. I’m looking to embed them into a website. Example: a 15 question list where they use sliders for input and then on the right it will show a temperature chart to reflect level of emotional regulation. Does anyone know a no-code tool I could use to achieve this? Thanks in advance!
