Why is collecting, managing, and taking action on customer feedback a challenge?

In my product management experience, I found the platforms we used required a lot of overhead in order to categorize and review feedback, to the point where we were not reviewing items regularly and missed opportunities to take action on high-impact, low-effort items. What challenges do you face in your approach to collecting, managing, and taking action on customer feedback?
